Specialized services are customized to meet individual, family, and community identified treatment needs in the least restrictive environment possible.

Catapult Wellness is proud to be an approved provider of Comprehensive Community Services (CCS) in both Waukesha and Kenosha Counties. Our CCS program offers individualized, recovery-focused mental health services, including psychotherapy, service facilitation, psycho education, skill development, and more. We’re committed to walking alongside individuals on their path to wellness.

Referral & Enrollment Information:

To enroll in CCS, individuals must first complete an intake through their county’s CCS program:

• Waukesha County Residents: Contact Waukesha County Health & Human Services at (262) 548-7666 to request a CCS screening.

• Kenosha County Residents: Contact Kenosha County Division of Aging & Disability Services at (262) 605-6646 to begin the CCS enrollment process.

Psychotherapy

Psychotherapy, or talk therapy, is a way to help people with a broad variety of mental illnesses and emotional difficulties. Psychotherapy can help eliminate or control troubling symptoms so a person can function better and can increase well-being and healing.

Catapult Wellness provides individual, family and group psychotherapy, in home or in our office We accept clients from age 6-60.

Care Coordination/Community Health Workers

A Community Health Worker (CHW) or Coordinator, is a community member of who empowers their peers through education and connections to health and social resources. They have proven success in increasing health outcomes for their communities because of thorough knowledge of the cultural norms of the people they serve, as they often live in the community themselves. They educate their peers about disease and injury prevention, work to make health services more accessible, and strengthen their communities to create positive change.

In these communities, access to health services is often restricted due to factors such as economic status, language, race/ethnicity, gender, age, geographic location, access to transportation, and availability of culturally appropriate health information. These barriers impact one’s ability to find affordable fresh foods, keep appointments with health professionals, adopt healthier habits, and apply for health care and services.

CHWs empathize with these complex life situations and are qualified to provide support to address these challenges by connecting individuals to community resources, organizations, and government programs that often provide free or low-cost services.

Home Visitation/Prenatal Care Coordination

Home visitation is available two programs:

Prenatal Care Coordination (PNCC) is a Medicaid and BadgerCare Plus benefit that helps pregnant women get the support and services they need to have a healthy baby. PNCC services include:

• Help getting the health care you need

• Personal support

• Information on good eating habits and health practices

• Help finding needed services in your community

SafeCare includes one-on-one home visits between home visitors (referred to as providers) and families. SafeCare includes three modules: (1) infant and child health, (2) home safety, and (3) parent-infant/parent-child interactions (Planned Activities Training).

We believe strongly in the Housing First Model and the need to provide permanent, supportive, housing in spite of circumstance. We support this initiative through programming that offers
